In order to participate in the LRHF 50/50 Staff Lottery, you must complete an opt-in form, information from previous lotteries will not be carried forward. Prizes may be claimed at the Lloydminster Region Health Foundation Office – Suite 116, 4910 50 Avenue Lloydminster, SK. Picture ID and Employee Card must be presented in order to claim the prize. All draws will be held at the Hospital Gift Shop – 3820 43 Avenue Lloydminster, SK. Draws will be held the Monday following Payday at 12:00pm local time. In the event the draw date falls on a Statutory Holiday, the draw will take place the following business day.
